Home Health Medical Record SpecialistSUMMARY OF JOB RESPONSIBILITIES:
- The Home Health Medical Record Specialist is responsible for the accurate and timely maintenance of all patient medical records, their safekeeping, and the confidentiality of all patient and employee information.
The Home Health Medical Record Specialist is directly accountable to the Business Manager, who is directly accountable to the Director of Home Care.
QUALIFICATIONS:EXPERIENCE:
- Minimum of one year experience :
- in Home Care or a similar healthcare setting is preferred.
- in Medical records/Record Keeping
- with Multiple Healthcare Software Applications, Microsoft Excel, Online Insurance Portals
- with the use of Printers, Fax, and Scanners
EDUCATION:
- High school diploma preferred
- Completion of Health Information Management classes preferred
- Professional experience is accepted in place of educational qualifications.
KNOWLEDGE AND SKILLS:
- Knowledge and skill required to perform the job responsibilities for technical and dimensional content.
- Must be able to operate office equipment and communicate on the telephone. Must be able to file accurately.
- Must be able to file, follow directions, complete assignments within given deadlines, and establish priorities.
- Proficiency in using a computer daily and basic IT problem-solving skills
PROBLEM-SOLVING/DECISION MAKING:
- The degree to which judgment and analysis must be exercised in planning, investigating issues, and evaluating alternate solutions posed by the job. It deals with the nature of the decisions, the variety of problems, and how decisions are made in the context of the typical job.
- Decisions are based on well-defined choices. Limited authority to make decisions or commitments directly related to the performance of the job.
SCOPE OF IMPACT/ACCOUNTABILITY:
- The potential impact of actions on results of BSMH and the job’s accountability for achieving results.
- Responsible for the proper use of supplies and equipment.
- Responsible for keeping confidential information regarding the agency, patients, families, employees, and any information in agency reports and/or files.
- The medical records clerk understands that improper release of information may be a direct violation of patient and employee rights.
- Must be accurate in filing, communicating information, and maintaining records and files.
- Realizes that errors may seriously affect patient care and the agency.
INTERPERSONAL COMMUNICATION:
- The nature and frequency of interpersonal human relations skills required by the job.
- The extent to which the job requires cooperation, meeting, or influencing others by phone, correspondence, or personal contact.
- Must be able to maintain a professional demeanor.
- Must get along with colleagues, patients, visitors, physicians, and other professionals/paraprofessionals.
- Responsible for promoting good interpersonal and inter/intradepartmental relationships.
- Must respect the dignity of each person, respond on a timely basis to requests for information, and create an atmosphere of tolerance and understanding.
PRINCIPAL JOB ACCOUNTABILITIES:
- Effectively maintains all patient medical records and is responsible for their safekeeping.
- Accurately and promptly obtain signed copies of orders, forms, and plans of care while keeping up with established standards.
- Establishes a unique medical record for each patient upon admission.
- Files all patient information accurately and timely, including nurses' notes, physician orders, and Form 485.
- When filing, update the Plan of Care Calendar to provide a record of all visits and related documentation.
- Verifies visits performed versus visits ordered and documents any discrepancies.
- Maintains confidentiality of patient and employee information.
- Demonstrates compliance with federal, state, and local rules and regulations, along with the HMHP Corporate Responsibility Program.
JOB ACCOUNTABILITIES – Other:
- Participates as a team member consistent with HM Home Care’s values.
- Willing to help other staff as needed.
- Serves as a backup for other office positions when necessary.
- Input patient data and information as required.
- Maintains established departmental policies and procedures, quality improvement programs, and safety, environmental, and infection control standards.
- Performs other duties as assigned and necessary.
- Promotes effective verbal and written communication in all internal and external interactions.
- Promotes a positive work environment.
- Communicate appropriate information regarding department operations to the Business Manager and/or Director of Home Care.
- As assigned, participate in educational programs, services, committees, and agency meetings.
