How to Fill Out the Korean Visa Application Form

The first section of the application form lists 4 guidelines for applicants:

  • Fill out the form completely and correctly.

  • Write in block letters (as in UPPERCASE letters) either in English or Korean.

  • For multiple-choice questions, check [√] all the options that apply.

  • If the applicant selects ‘Other’, provide more information in the given space.

Here are a few more tips to note:

  • Fill out the Korean visa application form using a computer. While you can submit a handwritten form, we recommend submitting a typed document for best readability.

  • For dates, use the format YYYY/MM/DD or 2023/11/10.

  • If there are items that are irrelevant to you, you may key in N/A.

  • Supply only true and verifiable information.

  • Print on A4-size papers.

  • Make sure to attach your photo on the first page and sign on the last page.

Sample Korean Visa Application Form

When I filled out my Korean visa application form, I downloaded the PDF file, annotated it on a PDF reader, and used Arial, size 16 pt. For the purposes of this blog, what you’ll see below is a sample with text responses in red, but be sure to use black color and at least size 16 pt when answering the document. (Note that some travel agencies may require you to use their preferred type of font.)

Let’s begin.

1. Personal Details

Now we're ready to go over the Korean visa application form step-by-step. The first part seeks to know your basic information. This should be straightforward.

Photo

The photo has to meet the following specifications:

  • 35mm x 45mm in size

  • Colored

  • Shows a white background

  • Taken within the last six (6) months

Moreover, your face should not be covered and must have a neutral expression.

1.1 Family Name and Given Names

Type in your last name and given name/s in the applicable fields.

1.2 Chinese Name (한자성ëȘ…)

If you have a legal name in Chinese characters, type it in. Otherwise, use N/A as I did.

1.3 Sex

Check the box (√) that pertains to your sex.

1.4 Date of birth

Use the format YYYY/MM/DD when you provide your birthday, i.e. 1992/11/10 for November 10, 1992.

1.5 Nationality & 1.6 Country of Birth

Write your nationality and country of birth in the next section. As for me, I put in Filipino and Philippines respectively.

1.7 National Identity No.

This section of the Korean visa application form asks for the National Identity Number. If you're an applicant in the Philippines who does not have a PhilID yet, just put N/A.

1.8 Has the applicant ever used any other names to enter or depart Korea?

If it’s your first time traveling to South Korea or did not have any changes to your name, tick No.

1.9 Is the applicant a citizen of more than one country?

If you have dual or multiple citizenships, make sure to tick Yes and provide the country/s that you’re registered with concurrently.

When you’re done with the Personal Details section, you can move on to the second page of the Korean visa application form. In case you’re unsure, you are NOT supposed to fill out the fields under For Official Use Only.

2. Details of Visa Application

2.1 Period of Stay (Long/Short-Term)

If you are applying for a tourist visa, put a checkmark beside “Short-term Stay less than 90 days”.

2.2 Status of Stay

Indicate the visa code for tourists, i.e. C-3-9 TOURIST.

3. Passport Information

3.1 Passport Type

Put a checkmark beside Regular. (If you’re curious, those who hold Diplomatic Passports are high-ranking government officials like the President, Vice-President, Senators, Members of Congress, Chief Justice, and some members of the Foreign Service, among others. Official Passports are given to any permanent or designated government employee for official travel.)

3.2 Passport No. & 3.3 Country of Passport

Your passport number is found in the upper right corner of the bio page. The latest version will have the following format: P1234567H

3.4 Place of Issue, 3.5 Date of Issue * 3.6 Date of Expiry

Find your passport’s place of issue, date of issue, and date of expiry at the lower part of the bio page.

3.7 Does the applicant have any other valid passports?

Put a checkmark beside No if you don’t.

4. Contact Information

This part of the South Korea visa application form requires your contact details, such as the following:

4.1 Home Country Address of the applicant

This is your permanent address in the Philippines.

4.2 Current Residential Address

This is where you stay or reside in the present. If it’s the same as your home country address, you may leave this blank or put N/A.

4.3 Cell Phone No. or Telephone No.

Type in your mobile OR landline number.

4.4 E-mail

Put your email address, preferably your primary.

4.5 Emergency Contact Information

The Emergency Contact can be your immediate family member, relative, or close friend. Ideally, this is someone whom the Embassy can easily reach in case you face an emergency. Provide their full name, country of residence, telephone or mobile number, and your relationship with them, i.e. spouse, child, father, mother, brother, sister, uncle, aunt, cousin, friend, etc.

5. Marital Status and Family Details

5.1 Current Marital Status & 5.2 Personal Information of the Applicant’s Spouse

If you are single or divorced, tick the applicable box and skip to item 4.3.

If you are married, check the applicable box and provide your spouse’s name, date of birth, nationality, address, and contact number. (If you will travel with your spouse, make sure to visit this Seoul landmark for a romantic view of the city!)

5.3 Does the applicant have children?

If your answer is Yes, put a check on it and indicate how many children you have. Otherwise, tick No.

6. Education

6.1 What is the highest degree or level of education that the applicant has completed?

Check the box that applies to you. If you have completed a diploma course or a vocational course, you can tick Others and provide the details on the space provided.

6.2 Name of School & 6.3 Location of School

Provide the name and address of the university, college, professional school, high school, or grade school.

7. Employment

The seventh section of the Korean visa application form seeks to know your present occupation, i.e. an employee, student, business owner, civil servant, or retiree, among others.

7.1 Current personal circ*mstances

Mark the box that describes your full-time or primary occupation.

7.2 Employment Details

Key in the name, address, and telephone number of your company, institute, or school.

8. Details of Visit

You’re down to the last 5 parts of the Korean visa application form! In the next section, you’ll be asked for a couple of information about your travel plans and history. Here’s how to fill out the required fields:

8.1 Purpose of Visit to Korea

Since you are applying for a tourist visa, put a check beside Tourism/Transit.

8.2 Intended Period of Stay

Indicate the number of days that you plan to stay in South Korea. Your response may be tentative.

8.3 Intended Date of Entry

Put in the date when you plan to enter South Korea. This may also be tentative. (Note that single-entry Korean visas are typically only valid for 3 months from the date of issuance. Hence, if you get your visa on November 15, 2022, your date of travel must fall anytime before February 15, 2023.)

8.4 Address in Korea

If you’ll travel to South Korea as a tourist, most likely you are thinking about booking a hotel, hostel, or shared home for your accommodation. Feel free to write the address of the property that you plan to book even though you have not made a reservation with them yet (A booking confirmation is not a requirement when you apply for a visa!). Of course, if you’ll be staying over at a friend’s or family’s house, you may also provide their address.

8.5 Contact No. in Korea

You can fill out this part with the mobile number that you have in the Philippines.

8.6 Has the applicant traveled to Korea in the last 5 years?

If you’re a first-time traveler to Korea, simply check the No response. However, if you have been to the country in the last five (5) years, be sure to indicate the number of times you visited, the dates, and the purpose/s of your visit. Here’s a sample:

8.7 Has the applicant traveled outside his/her country in the last 5 years except for Korea?

If you’ve traveled to other countries before in the last five years, type the name of the country in English, the purpose of visit, and the dates. Some reasons for traveling include tourism (leisure), transit, business, conference, study, medical tourism, and work.

In case you run out of space, you may use an extra sheet to list all the details of your trips in the past half-decade (exclude South Korea if you’ve already been!). Copy the table format provided in the application form.

8.8 Does the applicant have any family member(s) staying in Korea?

If you have a spouse, child, parent, or sibling who is staying in South Korea (most likely as a student, employee, entrepreneur, investor, etc.), note their information in this section. Otherwise, place a checkmark beside No.

8.9 Is the applicant traveling to Korea with any family member(s)?

If you and your family plan to travel to Korea together, provide their details accordingly. (In case you’re wondering, family member/s who will travel with you still need to fill out their own Korean visa application form and submit their own set of requirements even if you’ve listed their details here. Furthermore, the information of your relatives, boyfriend, girlfriend, or friends who’ll travel with you need not be supplied in this section.)

9. Details of Invitation

If you were invited by an individual or company based in South Korea, fill out this part. Put a check beside No if you don’t have anyone sponsoring your visa.

10. Funding Details

The second to the last part of the Korean visa application form seeks to know how you will pay for your travel costs, particularly your source of support, type of support, and estimated travel costs. Here are some guidelines when filling out this part:

10.1 Estimated travel costs in US Dollars

How much do you think you’ll spend in South Korea? Write the amount in US dollars. Logically, whatever amount you indicate here should be enough to cover your intended period of stay. Moreover, if you'll be paying for all your expenses, the bank statements that you’ll submit must show that you have at least as much money in your bank account.

To help you estimate your travel costs, you may want to consider the following items:

10.2 Who will pay for the applicant’s travel-related expenses? (Type of Support and Relationship to the Applicant)

As noted on the visa application form, your response can include yourself (the applicant), another person, and/or an institute. Since I’ll be paying for all my travel expenses, I put ‘SELF’ in the sample under Relationship to the Applicant. I also provided my name and contact number so I won’t leave any items blank. For the type of support, I put financial support.

If someone else is going to pay for all your travel expenses (e.g. company, a family member, relative, friend, etc.), supply their details. For the type of support, you can also put financial support.

If you plan to split the cost of your trip to Korea with other people, you may consider making use of a separate sheet to identify each person (or company) and specify what they’ll shoulder, i.e. Person 1 (Type of Support: AIRFARE), Person 2 (Type of Support: ACCOMMODATION), Applicant (Type of Support: MEALS AND TOURS). The goal here is to show that you either have the personal capacity or sources who will cover your trip expenses.

11. Assistance with this form

If you filled out the Korean visa application form by yourself, put a checkmark beside No. Otherwise, tick Yes and write the details and your relationship with the person who assisted you.

Moving on to the next section (page 5), you will see under Notice some terms from the Embassy of the Republic of Korea. Make sure to read each one carefully because affixing your signature denotes your agreement. Below are the terms that I annotated on:

  • If more space is needed to complete an item, record on a separate sheet of paper or submit relevant documents that will support the application. This may include a notarized affidavit stating that other people will pay for your trip, etc.

  • If you received a Korean visa and obtained a new passport afterward (due to loss or damage), you must notify the Embassy in Taguig or Consulate in Cebu about the changes in your passport information.

  • Possession of a visa does not entitle the bearer to enter the Republic of Korea upon arrival at the port of entry if he/she is found inadmissible.

  • C-visa holders (short-term visitors like tourists) will not be able to change their status of stay after their entry into the Republic of Korea. This is in accordance with Article 9(1) of the Enforcement Regulations of the Immigration Act.

  • Stating false information leads to the revocation of your visa and permission to stay. It may also result in criminal charges and an entry ban to the Republic of Korea.

  • Visa applicants may check their visa application status and results on the Korea Visa Portal website. Applicants will receive a disapproval or denial notice in written form when they collect their passport from a designated travel agency or the Consulate in Cebu.

12. Declaration

The last part of the Korean visa application form is the Declaration. Type in the date of your application and your name. You can then print out the document and sign it using a black ballpoint pen (I prefer this over placing my digital signature, even if you see one in the sample). If you’re a minor, ask your parent or legal guardian to sign.

How to Fill Out the South Korea Visa Application Form (5)

Tips

  • This guide is based on the South Korea visa application form available in the Philippines. If you will apply from India, Nigeria, Bangladesh, and other countries, make sure to download the application form available in your country.

  • Single-entry Korean visas are typically only valid for 3 months from the date of issuance. Hence, if you get your visa on November 15, 2022, your date of travel must fall before February 15, 2023.

  • Note that if you forget to provide your name and signature, your Korean visa application may be denied. Don't let your efforts go to waste, so make sure you review the form multiple times and check for typos, missed fields, etc. Make sure to attach your photo after printing.

  • Many applicants are curious if there’s a portion in the application form where they could request a multiple-entry Korean visa. There is no such section, but rest assured that your overall background (travel history, finances, the purpose of travel, etc.) will be considered to determine if you’re eligible to get one.
